How to Request a Copy of Your Medical Records/Images at WESTMED

How do I get a copy of my medical records and/or radiology images?

To obtain a copy of your medical records/images, the patient will need to fill out an Authorization for the Release of Medical Information form that is HIPAA compliant. When completing the form, please print clearly, sign/date, and include a complete mailing address. The completed form can be faxed to 914-682-6415 or emailed to medicalrecords@westmedgroup.com. If you are unable to fax or email the form you may mail to:

If the patient is a minor or is not competent to give consent, the signature of a parent, guardian or other legal representative will be required. Additional authorization may be required for release of specifically protected or privileged information. Contact the Health Information Management Department at 914-682-6416 with specific questions.

Some Helpful Questions and Answers 
 

Do I have to pay for my medical records?
Fees may apply. Here is a link to the form that details our fees.

Can my records be faxed to my physician or myself?
No. We are unable to fax records because we want to ensure the confidentiality of your medical information. However, we do offer encrypted email (file size limitations do apply) as an option. Just include the email address on the authorization form. Please click here for a tip sheet on how to open an encrypted email.

Can you release my records through the patient portal?
Limited information is available through your patient portal currently. We are not able to release additional information directly to your portal. If you have technical questions regarding the portal, please contact WESTMED technical support at 914-607-4717 or technical@westmedgroup.com.

What if I want my records to go to another doctor?
We do not charge to send copies of records directly to another physician for continuation of care. You still need to fill out an authorization form, and your records will be sent on a CD for the other physician’s convenience.

How long does it take to get my records?
All requests are worked in the order received. Processing may take up to 10 days from the date a valid authorization is received, however this is not typical.

Can I pick up a copy of my records?
We have a medical record representatives on site who can process small requests (15 pages or less) at any of the locations listed below. Please note that the patient must be present and show a photo ID.
