How do I get a copy of my medical records?
You need to fill out an Authorization for the Release of Medical Information that is HIPAA compliant. This can be found on our website under Information and then Patient Forms or click here. Please print legibly with a complete mailing address.

Do I have to pay for my medical records?
If you are requesting copies of your medical records for yourself, yes, it is $0.75 per page based on NYS Public Health Law 18. Radiology films are $15.00 per sheet. Mammograms are free per NYS Law. An Echocardiogram CD is $30.00. All radiology images on CD are $5.00

What if I want my records to go to another doctor?
As a courtesy, we do not charge for other physicians to receive copies of your records whether it is for a second opinion or you are transferring care. You still need to fill out an Authorization, however, your records will be send on a CD for their convenience.

How long does it take to get my records?
Normal processing time takes 2-3 business days. Film requests take 24-48 hours. CDs can be burned while you wait.

Can I pick up a copy of my records?
Yes. We have 2 locations for your convenience: 210 Westchester Avenue in White Plains, NY or 2700 Westchester Avenue in Purchase, NY. Our regular business hours are 8:30am-4:30pm. Films can still be picked up at 210 Westchester Avenue. Directions to both locations can be found here.

Can my records be faxed to my physician or myself?
No.
