The week of October 9 – 15 is National Case Management Week! WESTMED’s case managers play a unique role in the delivery of high quality and cost-effective healthcare services.
WESTMED currently has six case managers: Lisa Aaronberg, Deborah DellUomo, Patricia Elliott, Betsy Fruscillo, Linda Gengo and Ellen Wright. They assist our practice with meeting ACO and NCQA Patient-Centered Medical Home requirements by providing coordinated care to meet patients’ unique needs, collaborative clinical assessment, patient education, counseling, case monitoring, discharge planning and transitions of care, resource management and patient advocacy.
In addition to providing support to physicians who treat our patients with complex chronic diseases, our case managers also provide patient-family and community education on topics such as Positive Choices for Healthy Aging, Caring for the Caregiver, Caregiver Stress, Medicare & Medicaid Overview, When is the Right Time to Consider Assisted Living For You or a Loved One, What Do You Know About The Medicare Chronic Care Management Benefit.
