Medical Group has been selected by the Centers for Medicare & Medicaid
Services (CMS) as one of 516 awardees in 47 states, Puerto Rico, and the
District of Columbia to help reduce the risk of heart attacks and strokes among
millions of Medicare patients.
like WESTMED, in what is called the Million Hearts® Cardiovascular Disease Risk
Reduction (CVDRR) Model, will work to decrease cardiovascular disease risk by
assessing an individual patient’s risk for heart attack or stroke and applying
prevention interventions. Enrollment of members is slated to begin later this

tremendous potential to save lives and prevent debilitating diseases.
As a participant in
the program, WESTMED will work with Medicare patients individually to determine
their 10-year individual risk and to identify the best approach to reduce that
risk.” For example, smoking cessation programs, blood
pressure control, or cholesterol-lowering drugs or aspirin are options.
Dr. Morel added, “We’ll give each patient a personalized risk modification plan
that will target his or her specific risk factors.”
To determine patients’ risk, WESTMED will be using the ASCVD
(atherosclerotic cardiovascular disease) risk
